Please arrive at least 15 minutes before the appointment time to complete registration.

Please bring your insurance card and any photo ID with you. Patient Registration and Health Questionnaire forms may be downloaded and completed at home to speed the process.

Payment Options: If we are contracted with your insurance, we will bill them directly for our services. Copays are due at time of service. We accept cash, checks and most major credit cards except American Express.

For cash paying patients and motor vehicle accidents, we expect payment at the time of service.

Billing Questions: Call (425) 454-8191 during regular office hours.

Prescription Refills: To refill a prescription (after you are established with our clinic), please contact your pharmacy at least 3 days in advance. If you have not been seen within the past year, an appointment will be necessary. Antibiotic and narcotic refills require an office visit. Note mail order prescriptions usually require 2 weeks to process. Please bring any necessary forms to your office visit.

Cancellation of Appointments: Please call us 24 hours in advance so that we may offer that time to another patient. A “no show” fee may apply for failure to show up at scheduled appointment.
